What information should I have available?
When you call for an appointment, be prepared to give the following
information:
- Your health insurance member number,
located on your I.D. card.
- Your Primary Care Physician's name.
- Whether you are calling for an initial or
follow-up visit.
Do I always need to call in advance for
an appointment?
Yes. If you think you need a same-day appointment, please tell the
appointment representative so accommodations can be made.
